Reasons for referral:

* Habitual unruly behaviors within the home, school or community

* Lack of basic resources (i.e. food, shelter, transportation, medical, etc.)

* No current support services in place

* Significant emotional disturbances with youth/family

* Substance abuse/use

* Parenting and communication skill support

* Repeated social service interventions

* Law Enforcement contact (status/misdemeanor offenses)

*Cannabis Awareness and Prevention Class

 

Do you know a child or family that is struggling with mental health or behavior issues but you don’t know how to help them receive the services and supports they need?

Refer them to Youth and Family Community Connections and we will help them determine how to best to meet their needs.

Youth and Family Community Connections is a single point of entry assessment center for connections and supports to youth and families in the community, without involvement in the juvenile justice system.  Some examples of these connections and support would be social service referrals, coordinated entry into services, school connections for youth who are transitioning to different educational environments, and system navigators to help youth and families access the supports they need.  YFCC exists in the Youth Services Center building, which still houses the intervention services that currently exist, such as secure detention, shelter, and day treatment programming.

Youth and Family Community Connections partners with the various agencies already providing prevention services to ensure that youth are connected to the right prevention services to meet their needs. 

Training and education for youth, families and professionals can be provided or coordinated with existing efforts in the community. 

Leadership partners serve as an advisory to YFCC to ensure that we are meeting the needs of all youth in the community and collaborating with all youth servicing entities.
